Educational Background:
Received his Medical degree at University of Nigeria Teaching Hospital, Enugu, Nigeria in 1987.
Residency in Anesthesiology at Albert Einstein School of Medicine, Montefiore Medical Center, Bronx, New York in 1998.
Fellowship Training in Pain Management at University of Maryland, Baltimore in 1999.
Marital Status:
He is married and has 4 beautiful children. Resides in Phoenix and has been practicing in the Phoenix metropolis since 2005.
Hobbies:
He loves spending time with family and friends. Also, engages in outdoor activities such as hiking, swimming, running and golfing.
Personal Philosophy and Passion:
He loves the Lord Jesus from whom he gained and practices the Golden rule, “ Do to others as you will want them to do to you.”

About:
My name is Rachel Hauns and I am a Nurse Practitioner here with Izzy Pain Management. I have been with the practice for two years and am privileged to work with the caring staff here.
Goals:
My goal as a provider with this team is to provide the best possible treatment plan to serve you, our patient. My belief is that this is achieved through a trusting relationship. You, the patient, must trust my skill and judgment to treat your ailments and care for you, and I must trust the information you provide to develop a plan of care. I have been fortunate to find a place of work that approaches patient care in a holistic manner.
Educational Background:
I am an Arizona native and enjoy the winter months. I attended Grand Canyon University for both my Bachelors and Masters degrees.
Hobbies:
In my free time I enjoy spending time with my family as well as exercising. The Lord has richly blessed my life and my husband and I recently had our first child.

I am Daisy Lopez. I am from Mexico, and I grew up in Phoenix. Completed High School Diploma, went to Allen School Of Health Sciences to get my Medical Assistant diploma in 2014. Worked for Urgent Care and Behavioral health clinics. Love where I am at at the moment. I work with the most amazing people I ever met.
Hobbies:
I'm always busy with family, love to travel and try new things!

Edith attended University of Phoenix where she obtained her degree in Human Services. She also has a masters degree in Professional Mental Health Counseling from Grand Canyon University.
About:
Edith is a married mother of lovely children. Loves to spend time with her family.

My goal as a mental health professional is to provide a tailored approach to each client/patient. My belief in life is not to provide a one size fits all but to work with clients/patients to gain mental freedom and find healthy balance in life.

Enjoys ironing while watching Nigerian movies.
